Your Rights

~To be treated in a clean, safe and comfortable environment.
~Be treated with consideration, dignity and respect.
~Ask questions about your treatment
~Confidential treatment of your disclosed health information
~Stop a treatment at any time, for any reason
~Ask questions regarding staff training, licensing and certification.

~To be treated by staff who respectfully conduct treatments according to the clinics protocols, policies and procedures
~Without fear of retribution, ability to communicate your preferences, expectations and concerns

Your Responsibilities
~Communicate complete and accurate health information and reasons for visit.
~Treat staff and other clients with courtesy and respect.
~Use products, equipment and therapies as directed.
~Adhere to the clinics published policies and procedures


Please read the following policies and procedures that are enforced at our clinic. Should you have any questions or concerns regarding any of these. Please contact Caron@AestheticLaserandSkin.com and she will be able to either clarify or get your concern sent to the correct person.

Arrival
We ask that you arrive on time for your appointment. If you are late, we may have to shorten your appointment as to not punish or take time away from  the clients scheduled for the remainder of the day.
*Once 15 minutes late, you are considered a No Show, and penalties may apply. If you are running late, please give us a call, we might have an alternative.

Health Considerations
We do have a health questionaire that is required to be filled out entirely by every client. It is your responsibility to inform our staff of any changes to your health, or medications. As certain medications may be contraindicated for our services.

Children
The Medical Spa is not the appropriate environment for children, as they cannot roam freely, explore or be themselves.
We have items and instruments that can be harmful, and require that no children under the age of 12 be in attendance of any service at our Medical Spa at any time.Children 12 and over will have to be in the treatment room with you, please keep this in mind when scheduling your service. If you cannot find appropriate care for your child(ren), then we will have to reschedule your treatment.
Staff members are not permitted to watch or entertain children while on duty.
Please understand, that this is for the safety of the children, maintaining a calming atmosphere of the clinic as well as a courtesy to the other patrons.

Pets
No animals are permitted within the clinic. Due to allergies of the staff and clients, as well as sanitary considerations.
Service animals are always welcome.

Payment
All services must be paid for at end of treatment. For your convenience we accept Visa, MasterCard, American Express, Discover, Cash, Aesthetic Laser and Skin gift certificates, CareCredit, and all Debit cards with the Visa or MasterCard logo on them.
Gratuities have not been included into any service, and are accepted for a job well done.

Cancellations
Kindly give 24 hours notice, so we have an opportunity to fill the time slot.

No Show's for Injectables
All no shows for Botox or Juvederm treatments are required to post a $50 deposit in order to schedule in the future, this will be credited toward your treatment, assuming the appointment is kept. Should it prove to be a no show, then the deposit will be forfeited. Should this be habitual, we reserve the right to refuse service.

No Show / Cancellation / Reschedule Policy
It is the clients responsibility to know when the scheduled appointment is. We issue a reminder card when all appointments are scheduled in office. Send an email 2 days prior (if you have chosen to give us your email address), and offer a text message reminder 1 day prior to all appointments (if you have elected for this service as well). These courtesy reminders should provide you with enough time to make proper arrangements. Due to the demand of the treatment rooms and staff time, it is imperative that we enforce the policy to keep costs down for everyone.

Fee Schedule:
~Same Day cancellations (less than 10 hours notice) ~$35
~Same Day Reschedule to another day (less than 10 hours notice)~$35
~No show for any Scheduled service~ $50
~Late arrivals of 10 minutes may result in a shortened service (as to accomodate the next client in a timely fashion) More than 15 minutes late will result in appointment being classified as a no show and fee imposed. All fees must be paid before future appointments can be made. Please understnd that the technicians dont get paid if you dont show up,therefore if you cannot make your appointment... give them the courtesy to leave and run errands or schedule another client in your place. Failure to give appropriate notice is what results in the fee.
